Identification Requirements

All applicants must submit a copy of an accepted form of identification and a copy of proof of relationship (if required). After placing your order, you will need to upload an image of your identification and proof of relationship (if required). Your order will only be processed once a legible copy of identification and proof of relationship (if required) are received along with payment.

All applicants must provide a valid photo identification as required.

Valid and Current Photo Identification: Submit a copy of 1 of the following:

- Government Issued Driver's License
- Government Issued State ID
- Military ID
- Valid Passport/Visa

If you cannot provide any from the above list, you must submit at least 2 forms of secondary IDs from the following list:

- Social Security Card (must be signed by card holder)

- Bank Statement with Requestor's current address
- Car Registration or Title with Requestor's current address
- Utility Bill with current address of Requestor
- Current Pay Stub (must include your name, social security number plus name and address of business; not handwritten)
