After placing your order, you will need to upload an image of your identification and proof of relationship (if required). Your order will only be processed once a legible copy of identification and proof of relationship (if required) are received along with payment.

All applicants must provide a valid photo identification as required.

Valid and Current Photo Identification: Submit a copy of 1 of the following:

If you cannot provide any from the above list, you must submit at least 2 forms of secondary IDs from the following list:

- Letter from a government agency requesting a vital record
- Lease/rental agreement
