Identification Requirements

All applicants must submit a copy of an accepted form of identification and a copy of proof of relationship (if required). After placing your order, you will need to upload an image of your identification and proof of relationship (if required). Your order will only be processed once a legible copy of identification and proof of relationship (if required) are received along with payment.

All applicants must provide a valid photo identification as required.

Valid Photo Identification: Submit a copy of 1 of the following:

- State Government Issued Photo ID or Driver's License
- Bureau of Indian Affairs Issued Tribal ID Card
- U.S. Government Issued Military ID Card
- U.S. Government Issued Passport or Visa
- U.S. Government Issued Permanent Resident Card

If you cannot provide any from the above list, you must submit at least 2 of the following:

- Social Security Card
- Utility bill with current address (can not be more than 3 months old)
- Bank statement with current address (can not be more than 3 months old)
- Pay stub ( must include your name, SSN and the name and address of your employer)
- Car registration with current address (for the current registration year)
- IRS Tax Return (from the prior year)
